Today's episode discusses whether ADHD medications lose their effect over time and the effects of continued use. Based on a recent systematic review, we analyzed studies on tolerance and tachyphylaxis to these medications. Key points addressed: Therapeutic Effect vs. Subjective Effect: While tolerance to subjective effects (the feeling of being under the effect of the medication) may exist, tolerance to the therapeutic effect (improvement in attention, impulsivity, etc.) has not been consistently observed. Persistence of Effect: In most of the reviewed studies, the beneficial effects of ADHD medications are sustained over time, even at usual doses. Causes of the Perception of Loss of Effect: The perception that the medication "no longer works" may be related to factors such as dissipating subjective effects, fluctuations in the individual's life (stress, transitions), comorbidities, or problems with adherence to treatment. Long-Term Use: The need for continuous medication is individualized. Although ADHD is chronic, medication use should be evaluated for its safety and benefit, and may be complemented by the development of organizational and executive skills. Clinical Considerations: In clinical practice, the perceived decrease in effectiveness should be investigated, considering adherence, comorbidities, stressors, and, if necessary, dose adjustment, medication change, or strategies such as "drug holidays".
