In this condition the pain in the buttock region actually arises due to compression of sciatic nerve by a large muscle piriformis at the level of hip. This is in contrast to traditional disc prolapse and sciatica where the compression is at the level of lumbar spine. The treatment is mostly conservative with medicines and physio stretches. Intractable cases need injection of steroid and local anasthetic under xray guidance. only rare cases need surgery.
