Improving Diabetes Self-Management for Patients with Disabilities
Managing diabetes through effective self-care can be complex for patients with physical, sensory, intellectual, and developmental disabilities. Health center teams often encounter additional challenges related to communication, mobility, caregiver coordination, and access to services. This webinar highlighted practical strategies to strengthen patient engagement and improve care coordination. Participants explored workflow adaptations, caregiver education approaches, and collaboration strategies with public housing groups to support continuity of care.
