In this episode of the videocast "Polish Demo," Michał Tomasik (Polityka weekly) and Krzysztof Izdebski (Batory Foundation) speak with Dr. Maria Libura, representing the Collegium Medicum Olsztyn (University of Warmia and Mazury in Olsztyn) and the Polish Economic Network. The expert provides a detailed analysis of the current situation in healthcare and why previous reform attempts have failed to produce the desired results. The infamous scandal at the Southern Hospital serves as the starting point for a discussion of the structural weaknesses of the Polish healthcare system. The interviewees explain how this specific case differs from everyday problems and how such scandals in healthcare obscure the situation. Hospital financing and the complex ownership structure of public facilities are an important topic. The current fragmentation means that hospitals compete with each other instead of cooperating, which complicates care coordination, deepens the healthcare crisis, generates debt, and, in the longer term, forces hospital closures. The entire healthcare system in Poland is financially centralized through the National Health Fund, but responsibility rests on the shoulders of hospital directors, meaning that phenomena described as pathology in Polish hospitals stem from a flawed systemic framework, not necessarily the actions of staff. A good example is physician salaries and human resources management. As Dr. Libura explains, doctors in Poland struggle with the problem of multiple jobs and overwork. A full-time professor in a clinic earns an average salary, while the market competition to manage shift schedules creates enormous salary caps because, as the expert emphasizes, the system rewards mobility, not competence. The patient should be at the heart of the healthcare system, and the most valuable resource for the patient is the primary care physician they know. We also ask Dr. Libura about the ten commandments for overhauling healthcare – 10 commandments for politicians, which, in the authors' opinion, should become the foundation of real reform.
