Therefore, the FiO2 of the inspired air depends on how deeply the patient breathes through their mouth. It is a low-flow device that delivers a low concentration of oxygen. Due to the risk of drying the nasal mucosa, it is recommended not to deliver more than 3 liters per minute. It is useful for patients with mild desaturation and low minute ventilation. Oxygen Mask without Reservoir This device is becoming increasingly obsolete due to the availability of better and more efficient equipment. It is similar to the nasal cannula and can be used with up to 8 liters per minute, as it also utilizes the oral breathing route, thus avoiding drying the nasal mucosa. It is estimated that with 8 liters, it can deliver up to an FiO2 of 60% in patients with low minute ventilation. Oxygen mask with reservoir To understand the concept of the reservoir, it is essential to understand that the device delivers oxygen during both inspiration and expiration. This was not utilized in the previously mentioned devices, and that oxygen was expelled along with the exhaled gases. The reservoir bag causes the patient to inhale the gas contained in the bag, resulting in less ambient air being breathed with each inhalation in proportion to the pure oxygen. Thanks to this, higher oxygen concentrations are achieved compared to devices without a reservoir. There are two types of masks with a reservoir: those with a valve that prevents rebreathing, and those without. The rebreathing valve prevents exhaled air from entering the inspiratory bag, ensuring a higher FiO2 than in those without the valve. In both masks, the maximum flow rate that can be delivered is 15 liters/minute, theoretically achieving FiO2 of up to 80% in the rebreathing mask and up to 99% in the non-rebreathing masks. This decreases, as we already explained, when the minute volume is greater than the 15 liters delivered.
