For the treatment of gynecoid lipodystrophy, also known as cellulite, creams, massages, shock waves, radiofrequency, and subcisions can be used. We'll discuss all of this in this video. But first, let's take a quick look at the layers of the skin. The skin is divided into the epidermis, dermis, and subcutaneous fat. Cellulite is a structural and inflammatory change that affects the subcutaneous fat, or fat tissue. Connective tissue septa form in the skin, pulling or retracting it, causing dimples on its surface. In addition to the lack of microcirculation in this tissue, inflammation, fluid retention, and increased adipose tissue (the latter influenced by female hormones), there is also a risk of developing cellulite. Before continuing, I want to define three terms: Adipocytes: Cells that form adipose tissue, or fat. Lipolysis: Destruction of adipocytes Subcutaneous tissue: Deep layer of the skin where adipocytes are found Now we're ready to talk about treatments. TOPICAL TREATMENTS Caffeine, retinol, and some botanical compounds have potential effects such as lipolysis, lymphatic drainage, and stimulation of microcirculation. In general, they can improve the appearance and texture of the skin (called orange peel skin), but they do not improve skin retraction caused by fibrous septa. MASSAGES Mechanical stimulation through massage promotes lymphatic drainage and microcirculation, which decreases edema, thus improving the appearance of the skin. This can be done manually or with equipment. CHOU WAVES Commonly used in renal lithiasis, that is, to break up "kidney stones." These are acoustic waves that travel through connective tissue, promoting the breakdown of fat cells (adipocytes), improving local blood flow, lymphatic drainage, and reducing edema. RADIOFREQUENCY It emits thermal energy in the affected area due to the resistance of an electrical current flow in the dermis and subcutaneous cellular tissue, a phenomenon known as "electrical bioimpedance." The most recommended radiofrequency is unipolar, as it penetrates the deep layers of the skin, i.e., the cutaneous cellular tissue, and affects the connective tissue septa and adipose tissue. The mechanism of action of radiofrequency is as follows. It emits thermal energy, which is absorbed by adipocytes, causing the rupture of their membrane and, consequently, their destruction. This stimulates collagen production, improving the skin's appearance. Radiofrequency is usually combined with other treatments to achieve better results, such as ultrasound, infrared, and suction. SUBCISIONS Fibrous septa depress the skin (vertically downward), producing depressions on its surface. Therefore, there is a surgical technique that consists of breaking these fibrous bands or septa, releasing the retracted skin and thus improving its appearance. From this concept emerges guided and stabilized tissue subcision. A very innovative technique approved by the US FDA, the European Community (EC), and the Mexican Ministry of Health (SSA) that treats the underlying cause of cellulite. This system releases fibrous septa at a precise depth (6-10 mm), using a suction system and microblades. This technique has demonstrated long-term safety and effectiveness and is indicated for moderate to severe cellulite. Thanks for watching this video.
