SPECIALIZED PHYSIOTHERAPY Specialized Physiotherapy Manual Therapy Maitland Concept Treatment with accessory movements of the spine PAC - Central posteroanterior pressure PT - Transverse pressure PAUni - Unilateral posteroanterior pressure MAITLAND CONCEPT of MANUAL THERAPY The Maitland Concept is a technique of joint manipulation and mobilization, which can be performed in any joint, respecting the directions, ranges of movement, orientation of the joint surfaces and the anatomical individuality of each individual, efficiently applying the degree of mobilization appropriate to the stage of dysfunction, evolving the accessory passive mobilizations (arthrokinematics) and physiological mobilizations into active movements, with quality, with the great differential of the Maitland concept being Clinical Reasoning. It emerged in the 1960s, in Australia, and was idealized by Physiotherapist Geoffrey Douglas Maitland (1924-2010), co-founder of the IFOMT (International Federation of Orthopedic Manipulative Therapists), who developed his concept of evaluation and treatment. Currently, it consists of a standard and essential method in the professional life of physiotherapists who work with manual therapy. This concept is based on clinical data collected through a detailed anamnesis. It has been one of the pillars of modern manipulative physiotherapy. The Maitland Concept advocates that "Manipulation and Mobilization are not like a game of golf", that is, regardless of the physiotherapist's skill, joint parts can only be mobilized or manipulated in the directions in which they function; that the assessment must be done analytically, adding the information gathered in the subjective and objective examination to make the decision on the therapeutic procedure (Maitland 1986). Geoffrey Maitland's contribution to Manual Therapy was to classify the degrees of joint mobilization, so that they would have greater clinical applicability. Grade I: small-amplitude mobilization that does not reach the restrictive barrier; Grade II: large-amplitude mobilization that does not reach the restrictive barrier; Grade III: large-amplitude mobilization that reaches the restrictive barrier; Grade IV: small-amplitude mobilization that reaches the restrictive barrier; Grade V: small-amplitude mobilization done at high speed after the restrictive barrier, known as manipulation. Grades I and II are used in cases of pain, while grades III and IV are used when movement restriction is the main factor related to the symptom. The Maitland Concept recommends two ways of applying passive joint techniques: sustained or oscillatory, which can be used through physiological and accessory passive movements. The application of passive joint mobilizations provides the connective tissue with a mechanical response. Connective tissues such as skin, fascia, ligaments, tendons, joint capsules and muscle fascia are composed of distinct extracellular and cellular tissues, with different tension and load curves (Lederman 2001). The first task of the physiotherapist-investigator, according to the Maitland Concept, is to use clinical reasoning to identify which tissue(s) are compromised, and based on this decision, the application of the load: location, duration and amplitude are chosen. The Maitland Method is defined by the fact that its treatment is not based solely on a joint mobilization technique, but on a philosophy that encompasses evaluation and treatment, which advocates reasoning based primarily on clinical findings. Objectives of the Maitland Method This method is used for orthopedic treatment, helping to relieve pain and safely release joints, restoring normal range of motion, thus improving the function of the human body. The treatment is indicated for patients with neuromusculoskeletal disorders (peripheral and/or spinal joints may be involved, in addition to the temporomandibular joint – TMJ).
