The ProtecT trial is the most important study ever done in localised prostate cancer — but only if you understand who it actually applies to. After 15 years of follow-up on 1,643 men randomly assigned to active monitoring, surgery, or radiotherapy, the headline result was clear: survival was nearly identical across all three arms. But the trial enrolled almost exclusively men with ISUP 1 and ISUP 2 disease — meaning these findings apply to low and favourable intermediate-risk cancer, not to higher-grade disease.
