In this renal physiology lecture, Professor Zach Murphy explains the structure and function of the Distal Convoluted Tubule (DCT), a critical nephron segment involved in fine-tuning electrolyte balance, calcium regulation, and urine dilution. Building on Henle's loop, this lecture transitions from bulk reabsorption to targeted, hormonally regulated transport processes. We begin by outlining the key features of the DCT, which is divided into early and late segments. In the early DCT, sodium and chloride are reabsorbed via the Na⁺/Cl⁻ cotransporter, helping to further dilute the tubular fluid. This segment is impermeable to water, maintaining hypotonicity as the filtrate progresses toward the collecting duct. The DCT is also the primary site of PTH-regulated calcium reabsorption through apical calcium channels and basolateral Na⁺/Ca²⁺ exchangers. In the late DCT, we explore the role of aldosterone-sensitive principal cells, which begin to overlap functionally with the collecting duct. Aldosterone promotes sodium reabsorption and potassium secretion, while antidiuretic hormone (ADH) influences downstream water reabsorption depending on hydration status. Clinical correlations include the mechanism of action of thiazide diuretics, which inhibit the Na⁺/Cl⁻ cotransporter and promote mild diuresis while enhancing calcium reabsorption, helpful in conditions like calcium nephrolithiasis or osteoporosis.
