Introduction to the Ardens LTC Annual Recall System

This introductory webinar provides an overview of the Ardens LTC Recall System and how it supports GP practices to manage the full long-term condition (LTC) review pathway.The system enables practices to move beyond a simple invitation process, supporting a structured and proactive approach to planning, notifying, assessing, reviewing, and evaluating LTC care. Using a month-of-birth recall model, practices can coordinate reviews for patients with multiple conditions, reducing duplication and improving efficiency across the practice team.
