Ultrasound Diagnosis of Pericardial Effusion and Tamponade in the ED

The presentation covers the four fundamental cardiac views, with particular emphasis on the subcostal window as the traditionally superior acoustic window for effusion detection. Practical ultrasound guidance principles for pericardiocentesis are discussed, grounded in older evidence supporting high accuracy in emergency department effusion diagnosis. Rather than requiring hemodynamic instability, tamponade is presented as fundamentally a sonographic diagnosis. The lecture emphasizes that hemodynamic compensation may produce hypertension rather than the classically taught hypotension, shifting the diagnostic paradigm toward imaging findings.
