Cervical insufficiency is a shortening of the cervix of less than 25 millimeters or a dilation of the cervical canal of more than 10 millimeters. This diagnosis is made in the second or third trimester of pregnancy. The following patients are at risk for cervical insufficiency: — with abnormalities in the structure of the pelvic organs, such as a saddle-shaped uterus, a bicornuate uterus, or an intrauterine septum; — after gynecological procedures, such as hysteroscopy; — with cervical pathology, such as intraepithelial neoplasia; — with multiple pregnancies or polyhydramnios; — with extragenital diseases, such as collagenoses and Marfan syndrome; — with infectious and inflammatory diseases, including chronic endometritis. Cervical insufficiency is diagnosed using a transvaginal ultrasound of the cervical canal. If the problem is detected before 22 weeks of pregnancy, it is preferable to perform cervical suturing in a hospital setting. After this period, a cervical unloading pessary is used, whose shape corresponds to the anatomical structure of the female reproductive organs. The patient also takes vaginal medications, such as micronized progesterone or Utrozhestan. To undergo treatment with a cervical unloading pessary, the following must be taken: — a complete blood count; — C-reactive protein; — swab; — bacterial culture with antibiotic sensitivity; — PCR test for chlamydia.
