"We are human, we make mistakes." In medicine, however, this statement can have fatal consequences. An estimated 17,000 people die each year in Germany due to preventable errors in the healthcare system – that's more deaths than in road accidents. But why is the culture surrounding errors in German hospitals still often characterized by fear and silence? And how could these tragedies be prevented? Host Jasmina Neudecker meets Heinke Castagne, who has been battling colon cancer for five years. A supposed new tumor on her uterus turned out to be a tragic mistake: Despite the lack of laboratory results, the doctors initiated invasive procedures. The result: six operations, kidney infections, and a massive dependence on painkillers – all for a diagnosis that proved to be wrong. Why is it so difficult for doctors to admit mistakes? Jasmina Neudecker is searching for solutions for modern patient safety and visiting pioneers in this field: Ruth Hecker, patient safety officer at Essen University Hospital, is fighting against the "five most dangerous words in medicine" ("That could never happen to me") and establishing the Critical Incident Reporting System (CIRS). In Essen, over 450 errors are reported anonymously each year to learn from them instead of seeking scapegoats. Tobias Mühling from Würzburg University Hospital is focusing on high-tech in training. Medical students use virtual reality headsets to simulate the stress of the emergency room before they encounter real patients. And Christian Deindl, chairman of the German Alliance for Patient Safety, is calling for a change of mindset in politics. While countries like Denmark pay compensation from public funds and avoid lengthy legal proceedings, patients in Germany often have to fight for their rights for years.
