NO BASIC INSTRUMENTS (Reconstruction) AERO PERÚ Flight 603 Simulator: X Plane 11 Airplane: Boeing 757 version 2 Professional Flight Factor On October 2, 1996, a Boeing 757 with registration N52AW crashed. Three days before the accident, the aircraft was on a scheduled Lima-Mexico-Lima route. The plane began its flight from Peru to Mexico. Upon takeoff, a loud impact sound was heard in the left engine due to a bird strike. The flight continued normally, and upon landing in Mexico, after a visual inspection, it was ordered that an inspection be performed upon landing again in Lima-Peru, as the impact had damaged two turbine blades. Once landed in Lima, the plane was taken to the hangar for inspection. Maintenance was performed as ordered, and the plane also received a paint touch-up. Maintenance technicians usually covered the plane's static intakes with gray adhesive tape to prevent particles or dirt from entering them. The engine had already been repaired. After the repair, which was completed around 11:10 p.m. on October 1, the plane must be ready to pass supervision and quality control checks, as it was scheduled for a flight for passengers arriving from Miami at noon and awaiting a 757 to their final destination, Chile. After the work was completed, two shift supervisors and then a quality control supervisor inspected the plane, but none of the three found anything unusual. The plane was promptly towed to the apron and handed over to Captain Eric Schreiber, the plane's pilot, around 11:50 p.m., who, during the pre-flight inspection and with the aid of a flashlight, also observed nothing unusual. After the maintenance, 61 passengers, following a long layover, were finally called to the gate to board Flight 603 bound for Chile. The crew consists of seven flight attendants and two pilots. Eric Schreiber, the pilot in command, is 58 years old (21955), and David Fernández, the copilot, is 42. After some preliminary checks, Schreiber applied power and began his takeoff roll at 12:42 a.m. In the seconds before the rotation, Fernández noticed something unusual on his instruments. They had no instruments because the static ports were covered with tape, and they were ignored during the inspection. At 1:11 a.m. on October 2, 1996, an Aero Perú B757 crashed into the water at high speed 29 minutes after takeoff, causing all 70 occupants to suffer immediate fatal injuries. The accident is classified as a CFIT (Controlled Flight Into Terrain). The main cause of this tragedy was the obstruction of the aircraft's static ports by maintenance personnel.
