A TFCC (Triangular Fibrocartilage Complex) injury is one of the most common causes of ulnar-sided wrist pain – especially in sports (boxing, golf, tennis, etc.). In this video, as a physiotherapist, I'll show you what you can do starting today, why certain movements repeatedly trigger pain, and when conservative treatment is appropriate versus when arthroscopy should be considered (Pace et al. 2024). What happens in a TFCC injury? Many TFCC problems arise from axial loading (pressure from the wrist) on the ulnar side, especially when rotation (pronation/supination) or radial/ulnar deviation occurs simultaneously. This combination is typical in sports involving gripping, striking, supporting, or contact. Central lesions tend to have less regenerative potential (lower blood flow) – often, arthroscopic debridement is performed when surgery is indicated. Peripheral lesions are often more easily repairable – in these cases, arthroscopic suture/repair is more likely to be an option if surgery is indicated. (Pace et al. 2024) BUT: Most treatments are conservative, and this works very well! So, ensure a thorough diagnosis, intelligently manage stress, and consider the realities of the situation (season, goals, role/position, time constraints, expectations). In many cases, conservative treatment is the first step, but arthroscopy is planned for a timely date if it seems appropriate – because it can combine diagnosis and treatment. (Pace et al. 2024) What can you do specifically? 1) Trigger point management (immediately): For 1–2 weeks, reduce the combination of pressure + rotation + ulnar deviation (e.g., push-up variations, strong grip/pulling loads in rotation, supporting yourself on your hand). 2) Improve stability! Rehabilitation usually prioritizes grip strength, hand and forearm training, controlled rotation, and gradual load tolerance – instead of early maximum range of motion under pain. 3) Recognize clear signs of escalation: If pain/instability doesn't subside under everyday stress, the return to sports stagnates, or clear "catch" symptoms occur, the question of conservative versus arthroscopic treatment becomes more relevant.
