Food digestion begins in the mouth. When you swallow food, it is pushed down the esophagus, a muscular tube that carries food to the stomach where it is digested. From the stomach, food passes into the small intestine where digestion continues and nutrients are absorbed into the bloodstream. Your doctor may order a PEG feeding tube if you are unable to swallow, digest, or absorb food due to esophageal cancer, oral surgery, stroke, major surgery, trauma, burns, anorexia, pancreatitis, radiation therapy, or inflammatory bowel disease affecting the small intestine. Before the procedure, you will receive an intravenous sedative to help you relax. You will be given local anesthesia in the area of your abdomen where the PEG tube will be placed. The most common PEG placement procedure is the push method. During this procedure, the doctor inserts a lighted endoscope through your mouth, slides it down your esophagus, and into your stomach. A camera attached to the endoscope will produce images of the inside of your stomach, which will be displayed on a video monitor. The doctor will then insert a needle through your skin into your stomach where the PEG tube will be placed. While doing this, the doctor will use the endoscope to position the needle tip inside the stomach and encase it with a wire mesh. A thin wire will then be passed through the needle and into the stomach. The endoscope will be attached to the wire, and the endoscope and wire will be pulled out of the mouth. At this point, a thin wire will enter the front of the abdomen, into the stomach, and continue up toward the mouth. The doctor will attach the PEG feeding tube to the wire outside the mouth. Gently pulling on the other end of the wire, the doctor will withdraw the tube through the mouth, esophagus, and stomach. Continue pulling until the tip of the tube emerges from the incisions in the stomach and abdomen. A soft, round stopper attached to the portion of the tube inside the stomach will secure it in place. The outside of the tube will also be secured with a stopper, and sterile gauze will be placed over the incision site. In the alternative push method, the doctor will begin in the same way, using an endoscope to guide the tube through the abdominal wall into the stomach. However, instead of pulling the PEG tube through the mouth, the doctor will push it directly into the stomach over the tube. After the procedure, you will continue to receive fluids intravenously for one or two days. Once it is confirmed that your digestive tract is functioning, you will receive clear fluids through the PEG tube.
