F-22 vs KC-46 — What Went Wrong?

The Air Force has released its investigation into the KC-46 aerial refueling mishap with an F-22 refueling over the Atlantic. According to investigators, a combination of boom operator inputs and F-22 pilot errors led to the boom nozzle binding in the Raptor's receptacle. During the subsequent breakaway, the boom struck the KC-46's tail section, separated from the aircraft, and fell into the ocean.
