We also run training courses for other medical professionals around the world: This training video clip illustrates the role of 3D neurosonography in the diagnosis of open spina bifida (meningomyelocele) at 11-13 weeks of pregnancy. Intracranial translucency (IT) is the term proposed to describe the 4th ventricle of the fetal brain in midsagittal plane. Recently it has shown that absence of IT is associated with open spina bifida. The detection of IT has been proposed as a screening method for neural tube defects. In the cases of abnormal (absent) IT the diagnosis of open spina bifida can be confirmed by visualization of fetal brain abnormality. In fetuses with meningomyelocele at 11-13 weeks the intracranial collection of CSF is substantially reduced with a decrease in the area of the lateral ventricles and diameters of the third ventricle, aqueduct of Sylvius and fourth ventricle. This phenomenon is detectable by 3D neurosonography performed by transvaginal ultrasound. The brain of the 11-13 weeks fetus affected by spina bifida can be described as 'dried-up brain'. The sonographic features of 'dried-up brain' can include: Irregular shape of the lateral ventricles 'Expanded' choroid plexuses Collapsed 3rd ventricle Caudal and posterior displacement of the mesencephalon: CRASH SIGN Narrow aqueduct Not visible cerebellum Obliterated 4th ventricle The data presented in this ultrasound video clip are based on publication: Loureiro T et al.
