If you only focus on coughing it up without addressing the root cause, the symptoms will recur for years. 3 main groups of causes: 1️⃣ Nasal and sinus problems – postnasal drip Allergic rhinitis and chronic sinusitis cause mucus to drain down the throat, creating a feeling of phlegm blockage. This is the real cause of constant phlegm in the throat in many older adults, but it's often overlooked. 2️⃣ Gastroesophageal reflux disease (GERD) Not everyone with GERD experiences heartburn. Many people don't have typical symptoms, but acid and pepsin silently irritate the throat. If phlegm is heavy at night or after meals, this could be the real reason for constant phlegm in the throat. 3️⃣ Lower respiratory tract group – cigarette smoke, bronchitis Cigarette smoke increases mucus-secreting cells and reduces ciliary activity. Phlegm therefore becomes thicker and harder to expel. In long-term smokers, this is often the real reason for constant phlegm in the throat. Why does phlegm keep recurring? When the mucous membrane is irritated, mucus-secreting cells increase mucin production (MUC5AC, MUC5B). At the same time, cilia function less effectively. A vicious cycle forms: irritation → increased mucus secretion → throat clearing → increased mucosal inflammation → more phlegm. If the real cause of constant phlegm in the throat isn't identified, the condition will persist.
