Transcoccygeal approach (Wemm & Saberski, 1995): prone, mark midline under AP view first, then oblique/lateral; pierce posterior & anterior sacrococcygeal ligaments, confirm midline with contrast, look for reverse comma sign, then inject. Agents & RF: Diagnostic/therapeutic: local anesthetic ± steroid Neurolysis: phenol or alcohol (cancer pain focus) RF ablation: conventional RF ~70–80°C for ~90 s; may be combined with chemical neurolysis in select chronic pain cases. Coverage Gaps & Combo Strategy: Because gray rami communicantes end around L2 → superior hypogastric plexus, residual pain may persist after an impar block alone—consider adding a Superior Hypogastric Plexus Block in cancer pain for broader sympathetic denervation.
