This occurs because there is a weakening of a type of muscle surrounding the urethra that needs to contract to squeeze the urethra so that all the urine can escape. Some patients, however, experience weakness in these muscles, which can be triggered by several factors: Previous prostate surgery; Excess weight; Lifting too much weight; Lack of physical conditioning; Intestinal cold; Central nervous system problems; Chronic cough due to lung problems; All of these situations weaken this urethral muscle and cause urine retention. So, what is the solution to this problem? Resolving this problem involves strengthening the pelvic muscles through physical therapy. These exercises improve the contraction strength of the urethral muscle that surrounds the urethra, allowing the patient to eliminate accumulated urine. Another maneuver that can be taught to patients is to perform a "urethral milking." Just below the scrotum, place two fingers along its length and run them upward toward the penis to promote emptying of the retained urine. Other alternatives, such as using penis clips and collection systems, can also be tried. Using sanitary pads, especially at social events, is an alternative that saves the patient from potential embarrassment when urine tends to overwet underwear.
